If you choose to use your insurance to receive reimbursement, this means you will pay for sessions out-of-pocket and submit the statement to your insurance company for reimbursement. - What are my out-of-network benefits for behavioral health services?
- What is my deductible? How much of my deductible remains?
- How do I submit my claim forms and statements for reimbursement?
- Does my plan cover phone sessions and/or online sessions? If so, what is the coverage?
- How long will it take me to receive reimbursement?
- What is the out-of-pocket maximum for out-of-network services? Does it apply to behavioral health?
